New Graduates of Academic Medical Institutes

If you are graduating from an academic medical institute and will be completing your residency program, you may want to consider enrolling in the Medicare Part B Program after completion of your residency program.
All physicians and practitioners treating Medicare patients are required by law to submit the Medicare claim for their patient. In order to do so, the physician or practitioner should be enrolled in Medicare.
In order to enroll, completion of the appropriate Form CMS 855 application(s) is needed as indicated below.
Individual physicians or practitioners opening their own office requesting reimbursement under a social security number must submit a Form CMS 855I enrollment application.
Individual physicians or practitioners opening their own office as a professional corporation, professional association or limited liability company, which has a sole owner must submit a Form CMS 855I enrollment application.
Individual physicians or practitioners joining an existing entity as an employee or working as a 1099 contractor must submit a Form CMS 855I and Form CMS 855R enrollment application. The entity must be actively enrolled or must submit a Form CMS 855B enrollment application.
Individual physicians or practitioners opening their own office as a professional corporation, professional association or limited liability company, corporation, etc., which has more than one owner must submit a Form CMS 855B, Form CMS 855I and Form CMS 855R enrollment applications.
Medicare enrollment also requires the submission of a Form CMS 588 electronic funds transfer (EFT) authorization agreement. Submission of this agreement with the submission of a voided check or deposit slip will allow payments to be deposited directly into your bank account.
When submitting any Medicare enrollment application, suppliers should ensure all sections and fields on the application are completed and that the appropriate documents are attachments.
When enrolling, completion of a Form CMS 460 will ensure you are a Medicare Participation supplier. As a participating supplier, you accept Medicare payments as payment in full minus any coinsurance or deductible amounts. Payments will be generated to you instead of being sent to the patient and reimbursement is generally at a higher reimbursement rate.
