First Coast Service Options Inc. (FCSO) identified a processing issue with claims for patients flagged in our system as having Medicare as their secondary insurance. This issue affected some claims processed from October 1, 2006, through June 6, 2008. Services processed on and after June 7, 2008, processed correctly.

FCSO has identified the impacted claims. These claims will be adjusted to reflect Medicare as the primary insurance. Providers will receive revised Medicare Remittance Advice notices advising that adjustments have been made to the original claims.
