First Coast Service Options Inc. (FCSO) has identified a processing issue with claims and adjustments processed from July 24, 2008, through September 12, 2008. This issue affects certain surgery and assistant-at-surgery claims. Services processed on and after September 16, 2008, were processed correctly.

FCSO will adjust all affected claims systematically; no action by the provider is required. We ask that providers do not submit appeal or reopening requests and refrain from calling the customer service line regarding this processing issue. We apologize for any inconvenience this may have caused.
